Meldonium, sold as Mildronate, is a metabolic compound developed in Latvia in the 1970s. It blocks carnitine synthesis, which shifts cells from burning fatty acids toward burning glucose, the fuel that needs less oxygen. That is why it is used for both angina, a heart short on oxygen, and endurance, muscle short on oxygen at the redline. It is taken as 250mg capsules, usually 500 to 1000mg per day across a 2 to 6 week course. It is not a stimulant and you do not feel it acutely; the effect shows up as stamina and recovery. What meldonium is

Meldonium is a small metabolic molecule, trimethylhydrazinium propionate, developed at the Latvian Institute of Organic Synthesis in the 1970s and sold under the brand name Mildronate. Across Russia and the CIS it has been a standard prescription for decades, mostly for cardiovascular conditions like angina and heart failure. It is also the compound a generation of Eastern Bloc endurance athletes ran quietly, which is the reputation that eventually caught up with it.

How it works: it changes which fuel your cells burn

Meldonium blocks the enzyme that builds carnitine, the shuttle that carries long-chain fatty acids into the mitochondria to be burned. With less carnitine, cells lean on glucose oxidation instead. This matters because burning glucose produces more energy per unit of oxygen than burning fat. So under low-oxygen or high-demand conditions, the cell keeps making energy while using less oxygen. One mechanism, two applications: a heart starved of oxygen in angina, and a muscle starved of oxygen at the limit of exertion.

What it is used for

Two lanes. The established one is cardiac: decades of CIS clinical use as an anti-ischemic and cardioprotective agent, where the evidence is reasonably solid. The one most people come looking for is performance: endurance athletes report holding output longer and recovering faster, the same effort costing less. That second use is real in reputation but the formal evidence is CIS-weighted and has not been confirmed by Western systematic reviews. It is honest to hold those two lanes apart rather than let the cardiac data borrow credibility for the athletic claims.

Why it got banned

WADA added meldonium to the prohibited list on 1 January 2016, and a wave of athletes tested positive within weeks, the most famous being Maria Sharapova. The agency does not spend the effort to ban compounds that do nothing, so for anyone not in tested sport, the ban functions as an unusually strong endorsement that the compound affects performance. For anyone who does compete in tested sport, it is a hard stop: it will produce a positive test.

Dosing and where it fits

Standard performance dosing is 500 to 1000mg per day, commonly 250mg (1 capsule) taken 2 to 4 times daily, run as a 2 to 6 week course rather than open-ended. Take it earlier in the day, since it can be mildly activating and disturb sleep late. Because its effect is metabolic and not stimulant, it stacks cleanly with caffeine or modafinil on a hard day without adding jitter; it is the endurance base of a stack, not the acute kick. Treat it as a course-based stamina and recovery tool, judged over weeks, not a pill you feel in an hour.
